Medical Forms and Information Required at Registration

1. Certified Birth Certificate (needs to have the state seal and list name of parent who is registering student)   The hospital or mother’s copy is NOT acceptable.  If you submit your forms by fax or as attachments to an email, please include a copy of the birth certificate and bring the certified copy with you to Kindergarten registration.

2. Completed North Carolina Health Assessment - Kindergarten Students and Students who are enrolling into a North Carolina School for the First Time. 

Request this form from your child's physician. **NC Law requires a complete Kindergarten Health Assessment with 30 calendar days of enrollment in school. Failure to comply will result in student suspension with compliance required prior to re-admission.** 

3. Medication LogIf your student needs to take medication at school, you will need to print out and complete this form. Please note that the front side is to be completed by your child's doctor.

4. Immunizations Record- Your child must meet North Carolina Immunization Requirements for school entry. Below is a basic guide of the immunizations required for school entry.
